Prescribed Medicines at School

If your child needs to have medicine that has been prescribed by a doctor, during the school day, we will require you to complete a parental agreement to administer medicine. Please see the form below. (Alternatively, please ask for a form in the School Office.) We will only accept prescribed medicines that are in date, labelled, provided in the original container as dispensed by the pharmacist and include instructions for administration, their dosage and storage.
